Welcome to ERAU’s Student Health Insurance Plan, United Healthcare Student Resources (UHCSR). UHCSR provides comprehensive, affordable health insurance for our eligible students. Our plan complies with the Affordable Care Act (ACA). UHCSR is a Choice Plus Plan, which provides coverage for preventive care, hospitalization, surgery, maternity, emergency care, medically necessary physician care, prescription drugs and mental health.
All registered students must have health insurance (this does not include Worldwide Campus Students). International students cannot complete an online waiver. Please see below section on International Students.
If you already have insurance, you may waive out of the UHCSR plan. Dates to waive out vary by semester and are listed below. YOU MUST BE ENROLLED IN CLASSES TO COMPLETE AN INSURANCE WAIVER OR IT WILL NOT BE ACCEPTED.

UHCSR goes green! Students can set up an account online or put an app on their phones to get insurance information and ID cards. Students enrolled in UHCSR can request an ID card through United Healthcare Student Resources at www.uhcsr.com, go to Create Your Online Account Now, and follow the steps. The student must set up an account in order to request the card.
The United Healthcare Student Resources insurance rates are as follows for incoming students during these semesters:
Fall 2016: $1,248.00—Waiver begins July 8, 2016 through September 6, 2016
Spring 2017: $776.00—Waiver begins November 21, 2016 through January 18, 2017
Summer A 2017: $332.00—Waiver begins April 6, 2017 through May 18, 2017
Summer B 2017: $164.00—Waiver begins May 28, 2017 through July 6, 2017

At this time, only registered students of the Daytona Beach & Prescott campuses can participate in the UHCSR insurance. Worldwide students cannot participate in this insurance program.
United Healthcare Student Resources Goes Paperless! Everything is Done Online!
You can now do many things online, such as print out your own insurance card, change your mailing address, search for a network provider in your area, and check the status of your claims by setting up your own online account. It’s quick and easy!
If you do encounter difficulty, you can call Customer Service at 1-800-767-0700 (7 a.m. to 7 p.m., CST, Monday through Friday).
All International students and Embry-Riddle Language Institute (ERLI) students — by law and University requirements — must have health insurance as a condition of enrollment. This must be a United States-based insurance with major medical coverage and must meet residency requirements as established by the U.S. State Department. If you have insurance and wish to be exempt from the University group policy, the deadline for providing proof of alternative insurance is the end of the second week of classes.
To be exempt from the University group policy, please bring your policy provisions — stipulating the amount of coverage for illness and injury — to International Student Services located at the HUB, Building 273, Ste. 200, or call 386-226-6577.
Your information will be forwarded to the Health Insurance Coordinator, who will waive you out of the school insurance, and your account will be credited. If you do not provide proof of your alternative coverage by the second week of class, you will be responsible to pay for the University insurance policy.
For new and returning students to the ERAU campus—these are your waiver periods:
Summer A 2016: Waiver begins April 7, 2016 through May 19, 2016
Summer B 2016: Waiver begins May 26, 2016 through July 7, 2016
Fall 2016: Waiver begins July 8, 2016 through September 6, 2016
Spring 2017: Waiver begins November 21, 2016 through January 18, 2017
Summer A 2017: Waiver begins April 6, 2017 through May 18, 2017
Summer B 2017: Waiver begins May 28, 2017 through July 6, 2017
Please go to your ERNIE Account and once you are enrolled in classes you will see complete instructions in your TO DO List in ERNIE. If you see the message that the enrollment/waiver period is over, please contact Health Services for assistance.
Please note: The waiver needs to be completed once a year during the Fall waiver period if you are a continuing student. If you are entering in either Spring or Summer A&B for the first time, you need to complete a waiver during that term and again in the Fall waiver period.
International students and ERLI students:
Cannot complete the online waiver as described in the section below. International/ERLI students will have to see International Student Services in the College of Business, Suite 115, to waive out of the insurance plan. Questions? Call 386-226-6579.

Failure to waive the insurance by the semester deadline will result in the non-refundable insurance fee remaining on your student account. Embry-Riddle is not responsible for insurance waivers that are submitted after the deadline.

Enrollment with United Healthcare Student Resources includes a unique global emergency services program from Scholastic Emergency Services (SES). This program immediately connects you to doctors, hospitals, pharmacies, and other services if you experience a medical emergency while traveling 100 miles or more from your permanent residence, campus location, or in another country.
The preferred provider for this plan is United Healthcare Options PPO. If care is received from a preferred provider, any covered medical expenses will be paid at the Preferred Provider level of benefits. If the covered medical expense is incurred due to a medical emergency, benefits will be paid at the preferred provider level of benefits. In all other situations, reduced or lower benefits will be provided when an out-of-network provider is used.
