Health Requirements for International Students

- Medical Report Form — Required to register for courses
The Medical Report Form — along with your immunization history (English translation) — must be completed and returned to Health Services prior to enrollment.

Fax: 386-226-6082 - MMR (Measles, Mumps, Rubella) – (two doses) - Required
Immunization against measles, mumps, rubella is mandatory. Required: Proof of receiving two doses of MMR received on or after the first birthday. The second dose must be administered 30 days or more from the date of the first dose. Students may also opt to provide a laboratory report indicating positive titers for the diseases. - Hepatitis B (three doses) and Meningococcal Meningitis (one dose)
Although Embry-Riddle has never experienced an outbreak of bacterial meningitis, many college campuses have. Meningitis can be fatal. Survivors can be subject to amputations, seizures, deafness and strokes. Hepatitis B is a more insidious disease. Students can become infected and pass the virus to others before realizing they are ill. Students must either document the immunizations for Hepatitis B and Meningococcal Meningitis or complete a waiver. Embry-Riddle encourages all students to be immunized. Sponsored immunization clinics will be offered throughout the year for students missing immunizations. Learn more about these diseases:
Help Prevent Hepatitis | Voices of Meningitis- Tuberculosis Test - Required
Tuberculosis testing is required for international students enrolling from TB endemic areas. The test needs to be done within three months of entering the University. Students with positive results will be referred for a chest X-ray and additional care, if medically necessary. The T-Spot blood test can be obtained at ERAU Health Services. There is a fee of $55, which is charged to the student’s University account. Students may obtain a receipt to file for reimbursement through their health insurance plan or sponsor.
The policy is in force to protect you and the health of all students. Students who fail to comply will have a medical hold placed on their class registration and may be denied attendance, pending completion of the requirements.
